GUIDE
The Guiding an Improved Dementia Experience (GUIDE) Model is a new nationwide initiative from the Centers for Medicare & Medicaid Services (CMS) designed to improve the lives of people living with dementia and their caregivers. Launched in July 2024, this 8-year program provides comprehensive, coordinated dementia care to help individuals remain safely in their homes and communities.
What the GUIDE Model Offers:
- A personalized care plan and ongoing care coordination
- Caregiver education and support
- 24/7 access to a care team or helpline
- Respite services to give caregivers a breakHelp navigating community-based services
Who Is Eligible?
To qualify, individuals must:
- Have Medicare Parts A and B
- Not be enrolled in Medicare Advantage or hospice
- Live in the community or assisted living (not a nursing home)
While we are not a direct provider of the GUIDE Model, we partner with GUIDE-participating organizations to help ensure individuals and caregivers receive the respite and support services
